You must bring various documents to the office in order to finalise your file during a confirmation appointment with Dr Jean-Marc Puch.

The documents are :


The signed "ASSPRO" consents and information texts

The Oxford PTH and/or Oxford PTG + Womac PTG questionnaires completed according to the type of operation.


Authorisation for the removal of the femoral head during a PTH in duplicate.


The questionnaire concerning the Data Protection Act.


You will also need to have seen the anaesthetist with the dentist and the cardiologist.
