When you enter the clinic, you will need to bring various documents with you:
- Carte vitale - Identity card or passport
- Mutual insurance certificate or complementary CMU certificate, if you are entitled to one
- Triptych accident at work if necessary
- Consent forms (surgery and anaesthetist)
- Information sheet and confidentiality
- Results of recent biological examinations
- Blood group card
- Completed pre-anaesthetic questionnaire
- Prescriptions of the anaesthetist or surgeon
- Radiological file (X-ray, MRI, Scanner)
- Electrocardiogram (if requested by the anaesthetist)
- Personal medical treatment
You must also bring your belongings for one or more days depending on your hospitalisation, along with your toiletries.
If you also have English canes, support stockings, you must bring them with you when you come to the hospital, otherwise they will be delivered to you on the spot.
